地震伤的影像诊断价值

摘要
目的探讨地震伤的影像诊断价值。方法回顾性分析143例地震伤的影像学及临床资料,143例患者均行X线平片检查,78例行CT检查,19例行MRI检查。结果143例地震伤患者经影像学检查明确四肢骨折81例,单发骨折34例,多发骨折47例,开放性骨折6例,闭合性骨折75例。关节脱位14例,软组织内异物5例,膝关节半月板损伤1例,气性坏疽1例。脑硬膜外血肿4例,颅内血肿2例,脑挫裂伤9例,肾挫伤2例、肝挫伤1例。22例脊椎地震伤患者中,脊椎压缩骨折20例,爆裂性骨折2例。单发椎体骨折12例,多发椎体骨折10例,共累及31个椎体骨折。31个椎体骨折中,颈椎3个,胸椎12个,腰椎14个,骶椎2个。30例胸部地震伤均为复合伤,其中肋骨骨折18例,胸椎骨折8例,肩胛骨骨折3例,锁骨骨折3例,胸骨骨折1例,胸壁软组织伤5例,肺挫裂伤5例,肺内血肿1例,肺部感染4例,气胸及液气胸6例,双侧胸腔积液1例。结论影像学检查有利于地震伤的及时准确诊断和治疗方式的选择。
Objective To explore the clinical value of imaging diagnosis of the injury in earthquake.Methods Clinical and imaging data of 143 injury cases in earthquake were analyzed retrospectively.All the cases accepted X-ray plain film examination,and 78 cases accepted CT and 19 cases accepted MRI.Results Imaging examination could establish definitive diagnosis for 81 cases of limb bone fracture.The single fracture was found in 34 cases,multiple fracture in 47 cases,compound fracture in 6 cases,closed fracture in 75 cases.Moreover,dislocation of joint was found in 14 cases,foreign body of soft tissue in 5 cases,meniscus of knee joint in 1 case,gaseous gangrene in 1case,epidural hematoma in 4 cases,intracranial hematoma in 2 cases,contusion and laceration of brain in 9 cases,contusion of kidney in 2 cases and contusion of liver in 1 case.Among the 22 cases of the vertebral injury in earthquake,the vertebral compression fracture was found in 20 cases,and vertebral bursting fracture in 2 cases,single vertebral fracture in 12 cases,and multiple vertebral fracture in 10 cases.Among the 31 vertebral bodies of fracture,the fracture of cervical vertebra,thoracic vertebra,lumbar vertebra and sacral vertebra was found in 3,12,14,2 vertebral bodies,respectively.All 30 cases of the thoracic injury in earthquake were the combined injury.The fracture of rib were found in 18 cases,thoracic vertebral fracture in 8 cases,fracture of scapula in 3 cases,fracture of clavicle in 3 cases,fracture of sternum in 1 case,soft tissue injury of chest wall in 5 cases,contusion of lung in 5 cases,hematoma of lung in 1 case,pulmonary infection in 4 cases,pneumatothorax and hydropneumothorax in 6 cases and bilateral pleural effusion in 1 case.Conclusion Imaging examination is the most valuable examination method in diagnosis of the injury in earthquake,and it can not only make definitive diagnosis,but also play an important role in selection of therapeutic planning.
