针灸结合骨立冲剂对去卵巢大鼠骨钙代谢影响的研究

摘要
目的:通过用针灸、骨立冲剂、针灸结合骨立冲剂及西药尼尔雌醇对去卵巢大鼠骨质疏松症治疗后观察、比较,探讨针灸结合骨立冲剂对去卵巢大鼠骨矿化及参与骨钙代谢的激素、局部因子骨钙素(BGP)mRNA与骨形态发生蛋白-2(BMP-2)mRNA表达的影响,为针灸药物结合治疗骨质疏松症提供实验依据,以期为临床治疗骨质疏松症提供更为有效的方法。
     方法:将6月龄雌性SD大鼠去除双侧卵巢建立骨质疏松症模型,随机分为模型组、针灸组、骨立冲剂组(简称骨立组)、针灸结合骨立冲剂组(简称针药结合组)及尼尔雌醇组(简称西药组)。针灸组选取腧穴“大杼(双)”、“肾俞(双)”、“脾俞(双)”、“命门”,针刺30分钟后用艾条温和灸上述腧穴各5分钟。每日1次,10次为1个疗程,疗程间隔休息1天,共治疗6个疗程;骨立组用骨立混悬液灌胃,每日1次,疗程同针灸组;针药结合组则在骨立灌胃后再施针灸组方法治疗;西药组以尼尔雌醇混悬液每周灌胃1次,其余5天灌胃生理盐水,每周休息1天后进入下一疗程,治疗时间同针灸组;假手术组和模型组均用生理盐水灌胃,每日1次,疗程同针灸组。治疗期满后,分别检测下列指标:(1)分别用火焰原子吸收法、钼锑抗比色法测定腰椎骨骨钙、骨磷含量;(2)用双能X线吸收法检测大鼠股骨BMD及用三点弯曲实验测试大鼠股骨骨强度;(3)运用扫描电镜观察大鼠股骨松质骨骨小梁结构形态;(4)采用竞争放射免疫法(RIA)测定血清雌二醇(E_2)、甲状旁腺激素(PTH)、降钙素(CT)、骨钙素(BGP)含量;(5)利用RT-PCR技术检测大鼠股骨中BGP与BMP-2mRNA表达水平。
     结果:与假手术组比较,模型组大鼠骨钙、磷含量及骨密度极显著下降(P<0.01),骨强度明显减弱(P<0.01),骨微结构破坏,子宫萎缩明显(P<0.01),血清E_2、BGP、CT含量极显著降低(P<0.01),PTH含量极显著升高(P<0.01),骨组织BGPmRNA、BMP-2mRNA表达明显减弱(P<0.01);通过针灸、骨立及针药结合治疗后,与模型组比较,骨钙、磷含量及骨密度、骨力学参数值不同程度升高(P<0.05或P<0.01),骨微结构不同程度改善,子宫萎缩得到不同程度控制,血清E_2、BGP、CT含量极显著增加(P<0.01),PTH含量极显著下降(P<0.01),股骨BGPmRNA、BMP-2mRNA表达明显增强(P<0.01);针药结合组各指标值与针灸组、骨立组比较具有显著性差异(P<0.05或P<0.01),与西药组比较除子宫重量、血清E_2、PTH、CT及股骨BGPmRNA、BMP-2mRNA具有显著差异(P<0.05或P<0.01)外,骨钙、磷、BMD、骨力学参数值均无差异(P>0.05),且骨小梁结构形态相似。
     结论:(1)绝经后骨质疏松症模型复制成功;(2)针灸、骨立和针药结合均能促进去卵巢大鼠骨质有效的矿化,改善骨微结构及增强股骨强度;(3)针灸、骨立和针药结合均能调节去卵巢大鼠参与骨钙代谢的激素E_2、PTH、CT的表达,刺激骨形成,抑制骨吸收,促进骨钙代谢平衡;(4)针灸、骨立和针药结合均能促进去卵巢大鼠股骨组织BGPmRNA、BMP-2mRNA表达,使骨钙代谢趋于平衡;(5)针药结合对去卵巢大鼠骨钙代谢的调节作用明显优于单纯针灸、骨立的作用。
Objective:To investigate the combinative treatment of acupuncture and Gu-li' s affections on ovariectomized rat' s bone mineralization、hormones involved in the bone metabolism and the expression of BGP mRNAand BMP-2 mRNA as well,the research was designed to observe and compare the results of treatments for ovariectomized rats' osteoporosis by using acupuncture and moxibustion treatment、Gu-li、combination of acupuncture and moxibustion and Gu-li、nilestriol in the hope of providing an experimental basis for the combinative treatment of acupuncture and herb and an effective clinical treatment on osteoporosis.
     Method:Female SD rats of six months were established to osteoporosis model by removed the ovaries under anesthesia were randomized into model,acupuncture and moxibustion treatment,Gu-li,combination of acupuncture and moxibustion and Gu-li and nilestriol groups,acupuncture and moxibustion treatment group which received only acupuncture and moxibustion therapy and combination of acupuncture and Gu-li group which received acupuncture and moxibustion plus Chinese herb Gu-li powder therapy.In the two groups,bilateral "Dazhu"、"Shenshu"、"Pishu" and "Mingmen" were punctured with acupuncture needle and fumed with moxa stick once daily and 6 treatment periods altogether.Rats of Gu-li group were drenched with Gu-li powder once daily and nilestriol group was drenched with nilestriol once weekly.After trentment,blood samples were collected from femoral artery.(1)Bone calcium and phosphonium of the lumbar vertebra by using flame atomic absorption method and moly-stibine anti-colormetric method respectively.(2)Bone mineral density of the femur of the rats were detected by using dual energy X-ray bone density instrument and bone strength detected by three-spot bending test.(3)Structure-morph of femur cancellous bone trabecula was observed by sCanning electron microscope.(4) Serum E_2、PTH、CT、BGP were assayed by using radioimmunoassay.(5) Expression of BGP and BMP-2 gene were examined with the method of RT-PCR in bone tissue of osteoporosis rats.
     Result:After the rats had been removed the ovaries,bone calcium and phosphonium and bone density lost and nature of bone vitodynamics depressed obviously,shapes of bone trabecula were thinner and quantities were decreased,uterus weight depressed obviously,content of serum E_2、CT、BGP degraded and PTH advanced significantly,BGP and BMP-2 gene expression been weaker than sham group(P<0.01).Metratrophia was controlled after treatment with the method of combination of acupuncture and Gu-li、acupuncture and Gu-li,serum E_2、CT、BGP raised obviously and PTH decreased moderately,BGP gene advanced and displayed positive correlation compared with serum BGP and BMP-2 gene heightened too,bone calcium and phosphonium and BMD and index bone vitodynamics increased at different degree.combination of acupuncture and Gu-li group surpassed significantly acupuncture treatment and Gu-li group from results of statistical analysis of effects on serum E_2、CT、PTH、BGP and BGP and BMP-2 gene and bone calcium and phosphonium、BMD and index bone vitodynamics(P<0.05 or P<0.01).
     Conclusion:(1)Model of osteoporosis was successfully duplicated. (2)Acupuncture and moxibustion、Gu-li powder、acombination of acupuncture and Gu-li can all improve and normalize bone matrix mineralization、improve bone microarchitecture and reinforce femoral bone strength.(3)Acupuncture and moxibustion,Gu-li powder、combination of acupuncture and Gu-li can all adjust the expression of hormone of E_2、PTH and CT which participated in bone calcium metabolism.(4)Acupuncture and moxibustion、Gu-li powder and combination of acupuncture and Gu-li can all promote the expression of ovariectomized rats' femur of BGPmRNA and BMP-2mRNA.(5) The treatment of acupuncture and moxibustion combined with Gu-li powde is obviously superior to single-method therapy of acupuncture or Gu-li.
