基于传统中医针灸理论的脑血管疾病物理治疗系统研究

摘要
急性脑血管疾病或卒中(中医又称中风)是危害人类健康的常见病,具有高发病率、高死亡率、高致残率、高复发率的特点。现代医学对脑血管疾病尚无明确高效的治疗方法,临床上许多治疗带有经验性、试验性。研究表明,综合采用内科、外科和物理治疗等多种方法,中西医结合是治疗脑血管疾病的最佳选择。
     中医针灸治疗中风病有几千年历史,疗效确切,费用低廉正被世界上越来越多的人所接受。因此,研究基于中医针灸理论的物理医疗仪器具有重要的理论和现实意义。本文以中医针灸治疗中风病的理论和经验为基础,研究基于传统中医针刺手法和艾灸疗法工程复现的中风病物理治疗系统。主要内容有:
     采用虚拟仪器技术,基于PC平台,设计以醒脑开窍法和头区刺激治疗理论为医学基础的以电针补泻、光灸补泻为核心技术、带有智能仪器特征和专家知识帮助功能的脑血管疾病物理治疗系统。
     仪器系统中电针的关键技术研究。通过对传统中医针刺补泻手法和补泻实质的研究,对针刺补泻手法进行特征提取,研究基于符号化测量理论思想的针刺手法特征域向工程域映射的仿传统针刺补泻手法的电针补泻针法信号设计与实现方法。研究基于模糊分区和混沌序列调制解决电刺激耐受性问题的方法与实现。
     仪器系统中光灸的关键技术研究。基于色温原理的间接测量和采用自建系统研究传统中医艾灸热辐射光谱特性和200-15000nm波段热辐射能量分布。研究采用卤钨灯加滤光片实现传统艾灸热辐射光谱工程复现和根据人体动态深层刺激理论的光灸补、泻方法。
     文中还对本系统采用的贴片电极作用下电刺激皮肤模型的建立与仿真进行了研究,从理论上初步说明贴片电极电针实现动态深层刺激的科学性。
     最后,通过动物和人体实验验证本文设计的系统用电针和光灸模拟传统中医针灸补泻效应的可行性,从而为脑血管疾病的治疗提供疗效的保证。
Acute Cerebrovascular Disease,(ACVD) or stroke is a kind of popular disease that harming the human health with high incidence, death rate, cripple rate and high recrudescence rate. In modern medicine, there isn’t yet a reliable and efficient curing method for this disease. Most of the clinical treatments of stroke are experimental. According to the result of research, the most efficient treatment method of stroke is the combination use of internal medicine, surgery and physical methods.
     Acupuncture and moxibustion in Traditional Chinese Medicine (TCM) has been applied to treat stroke for thousands of years. By now, its efficacy has been proved by clinical practice and is being accepted by more and more people in the world. It is significant to study the theoretical basement of therapeutic instrument that based on the TCM Acupuncture and moxibustion.
     In this thesis, the author aimed at a stroke physical therapeutic system based on the engineering duplication of the needle manipulating methods and moxibustion in TCM. The main works are as following.
     Design of therapeutic system basing on the personal computer and the medical theory of XingNaoKaiQiao and Cranial Stimulation. The core technique is the electroacupuncture and optic moxibustion reinforce and reduce. The system has the characteristics of intelligent instrument and expert knowledge help function will be also concerned.
     Study on the critical technique of the system-electroacupuncture. Basing on the study of the needle manipulating methods in TCM, we extract the features of the needle manipulating methods of reinforcement and reduction, and then map these features from the field of TCM to engineering field according to the symbolic measuring method. Study on the corresponding electroacupuncture signals that simulating the needle manipulating methods of reinforcement and reduction in TCM. Study on methods of electroacupuncture reinforce or reduce signals basing on the series of chaos and fuzzy distribution to solve the adaptability problem in electric stimulation.
     Study on the critical technique of the system _optical moxibustion. The study and measurement of the heat radiate spectrum of moxibustion in TCM. These works includes : the indirect measuring methods basing on chroma principle; Direct measuring using a self constructed system The calculation and estimation of the 200-15000nm spectrum and energy distribution of moxibustion. Study on the Spectrum duplication method of moxibustion by using a tungsten-halogen lamp with an ellipsoid reflex housing and optical filter. By PWM control of power of the light radiator, realize optical moxibustion reinforce and reduce according to the theory of active and deep stimulation.
     Study on the model establishment and simulation of skin under electric stimulation to validate the active and deep stimulate by plane electrode.
     By human and animal experiments, study on the feasibility of the electroacupuncture and optical moxibustion in the system designed that simulating the reinforcement and reduction of acupuncture and moxibustion in TCM. This may prove the efficacy of the system in treating stroke.
