摘要

目的:在上海市新一轮社区综合改革背景下,马陆镇社区卫生服务中心通过对医养结合服务需求侧及供给侧的调查和分析,建立了基于社区卫生服务中心实际供给能力、服务内容和形式各不相同的居家、社区、机构养老医养融合服务模式,通过一段时间的运行,社区卫生服务中心平台与养老服务体系有效衔接整合,社区卫生服务中心医养结合项目初步界定,医养结合服务补偿补贴机制初步建立。马陆镇社区卫生服务中心通过医养结合服务项目推进,为辖区内不同养老形式的老人提供医疗服务,初步改善了辖区内"医养分离"问题。
        In the context of the new round of comprehensive reforms in communities of Shanghai,the Malu community health service center established the actual supply capacity,aged care modes of families,communities and institutions in various forms and contents based on investigating and analyzing the supply side and demand side of intergrated aged care.Through a period of operation,the community health service center platform and the aged care service system were effectively linked and integrated with each other,and the community health service center medical care integrated project was initially defined,the medical care combined service compensation mechanism was initially established as well.The Malu community health service center promoted through the medical care integrated service project to provide medical services for the elderly with different forms of aged care in the area,and initially improved the problem of "separation of medical care" within the jurisdiction.
