实时、同体监测岭南火针对带状疱疹后遗神经痛模型大鼠外周神经递质的影响及其时效性

目的:探讨不同干预措施(岭南火针、电针治疗)对神经性疼痛的作用机理及其时效性.方法:32只雄性SD大鼠随机分为正常对照组、模型对照组、电针治疗组和岭南火针治疗组,于大鼠右足蹼底部皮下注射体积为50μL的带状疱疹病毒接种溶液,利用微透析技术,同体、实时、在线监测岭南火针、电针治疗对带状疱疹后遗神经痛模型大鼠外周腺苷及其代谢产物质量浓度的影响.结果:造模后,各组大鼠外周组织内腺苷、AMP、ADP质量浓度降低,治疗后30～60 min,电针组局部腺苷、AMP质量浓度接近基线水平,与模型组相比有统计学差异(P<0.01),治疗后90 min,电针组大鼠局部腺苷、AMP和ADP质量浓度逐渐下降至模型组水平(P>0.05).岭南火针组镇痛效果起效时间在60 min,治疗后120 min,外周组织中腺苷、AMP、ADP仍处于基线状态,与正常对照组无显著差异(P>0.05).岭南火针治疗组与电针治疗组相比,有统计学差异(P<0.01).结论:电针在治疗后30 min即能够起到镇痛效应,岭南火针在治疗后60 min起效,且能至少维持2 h,岭南火针较电针组而言,具有镇痛效果持久的优点.
Objective:To investigate the mechanism and timeliness of different methods(Lingnan fireneedle and electroacupuncture) on neuralgia. Methods: Thirty-two male Sprague-Dawley(SD) rats were randomly divided into normal control group, model control group, electroacupuncture group and Lingnan fire needling group. 50 μL inoculation solution of herpes zoster virus was subcutaneously injected into the plantar area of the right hindpaw of rats to make the model of PHN. Real-time online monitoring was performed by using microdialysis to investigate the effect of fireneedle and electroacupuncture on concentration of peripheral adenosine and its metabolites in rats with postherpetic neuralgia.Results: The level of adenosine, AMP and ADP in peripheral tissues of rats in each group was decreased after modeling. At 30-60 minutes after treatment, the topical level of adenosine and AMP in the electroacupuncture group was close to the baseline level, which was of statistically significant difference from model group(P<0.01). The level of adenosine, AMP and ADP in the electroacupuncture group gradually decreased to the level in model group at 90 min after treatment(P>0.05). In Lingnan fireneedle group, the onset time of analgesic effect was 60 min after treatment, and the peripheral level of adenosine, AMP and ADP was still at baseline at 120 min after treatment, which had no significant difference from normal control group(P>0.05). There was statistical difference between electroacupuncture group and Lingnan fireneedle group(P<0.01). Conclusion:Electroacupuncture makes an analgesic effect from 30 min after treatment, while fireneedle from 60 min and lasts 2 hours at least. Lingnan fireneedle has the advantage of lasting analgesic effect when compared with electroacupuncture.
