肾外肾源性残余2例并文献复习

摘要

目的探讨肾外肾源性残余(ENR)的临床病理特点。方法分析2例ENR的临床特征、组织学形态及免疫表型,并复习相关文献。结果 ENR无特殊的临床表现,常因其他疾病入院体格检查时发现病灶,部分病例可以出现在畸胎瘤内。显微镜下观察,大量成熟性纤维性间质背景下,可见肾小管、肾小球样结构,并见少量肾基质,未见明显核异型性及核分裂相,未见包膜或假包膜结构。增殖性肾源性残余病灶内肾基质成分较多,肾基质内细胞密集、重叠,缺乏胞浆,核染色质细颗粒状,未见明显核仁,核膜光滑,见较多核分裂,但缺乏细胞异型性。肾基质表达WT-1,P53,Ki67增值指数较高。结论 ENR为良性病变,手术切除后,预后好,但有少数病例,特别是增殖性肾源性残余病例可能会复发、转移,甚至进一步发展为肾母细胞瘤,需要长期随访。
