灾难救助者替代性创伤问卷的编制及初步应用

摘要
替代性创伤的研究始于上世纪九十年代,广义的替代性创伤可以理解为非直接遭受创伤伤害的人们以任何方式接触到创伤性事件而引起的不适反应。其中,灾难救助者群体的替代性创伤研究倍受国内外学者的关注。鉴于目前国内相关实证性研究较少,且缺乏标准化测量工具,编制适用于我国灾难救助者的替代性创伤问卷,有着重要的理论意义和实践价值。
     本研究旨在编制灾难救助者替代性创伤问卷,并初步探讨救助者替代性创伤的基本特征。在开放式访谈和文献综述的基础上,采用问卷调查的方式对哈尔滨、北京、唐山、保定等地211名5.12地震救灾志愿者进行测量。通过项目分析、相关分析、因素分析等方法检验问卷信、效度,并通过方差分析等方法探讨替代性创伤人口学变量上的差异,主要得出以下结论:
     灾难救助者替代性创伤由心理和生理两部分组成,心理部分由认知反应、情绪反应、行为反应和生活信念四个因素组成,问卷具有良好的信、效度;灾难救助者替代性创伤在年龄、职业、是否曾接受过专业培训以及工作历时等变量上存在显著性差异,在性别变量上仅行为反应因素存在显著性差异;心理工作者和医护人员中,志愿者与非志愿者替代性创伤无显著性差异,而在学生、媒体工作者及其他职业工作者中,志愿者替代性创伤表现程度较非志愿者严重。
Vicarious traumatization refers to a set of negative responses of individual who are exposed to traumatic events by any ways. Particularly, the trauma helpers were paid close attention by professionals. Due to the lack of relevant empirical research and standardized measurement instruments in China, the purposes of this study were developing vicarious traumatization questionnaire and exploring the different demographic characteristics of vicarious traumatization on trauma helpers as well. Based on interview and pilot study, the research subjects who were 211 trauma helpers to Si Chuan after "5.12 earthquake" were investigated.
     The questionnaire included two parts of psychological and physiological, and through item analysis and factor analysis, the psychological part were found of four dimensions: cognitive response, emotional response, behavioral response and life beliefs. The reliability and validity of those five subscales were also established.
     By the results of ANOVA, the major results were:there were significant differences of vicarious traumatization of trauma helpers on age, occupation, professional training and lengths of helping hours,as the same as that on behavioral response of gender. In addition, trauma participant helpers tend to suffer from more vicarious traumatization than non-participant helpers among students, media workers and other professional helpers, but it was not found in both mental helpers and healthcare helpers.
