WU多瘤病毒所致小儿呼吸道感染的临床特征研究

摘要
目的探讨WU多瘤病毒(WU polyomavirus,WUPyV)所致小儿呼吸道感染的临床特征。
     材料与方法
     材料:收集2008年1月至2010年9月汕头大学医学院第二附属医院儿科1701例因呼吸道感染住院患儿咽拭子,并分别收集WUPyV感染和RSV感染患儿年龄、性别、症状、体征等临床资料。
     方法:1、应用多重PCR技术,检测WU多瘤病毒(WUPyV)、呼吸道合胞病毒(RSV)、人博卡病毒(HBoV)、腺病毒(ADV)、鼻病毒(RhV)、流感病毒A、B型(FluA、FluB)、副流感病毒1、3型(PIV1、PIV3)、人类偏肺病毒(hMPV)10种(型)病毒。
     2、对WUPyV阳性病例的临床资料进行分析,并与RSV阳性病例的临床资料进行分析比较。
     结果
     1、1701例患儿标本中检测阳性1215例(71.4%),其中WUPyV阳性46例(2.7%),RSV阳性365例(21.5%);两者单纯感染分别为22例(47.8%)和88例(24.1%)。
     2、WUPyV阳性检测率在春季呈现高峰,RSV则在冬季出现高峰;WUPyV感染的患儿的平均年龄为26.23月龄,较RSV感染的患儿大。
     3、WUPyV感染患儿的临床特征与RSV感染相似,但咳嗽和喘息在RSV感染患儿中更为多见;两者在肺部体征、WBC计数、CRP值、住院天数无明显区别。WUPyV感染后所致的疾病主要是支气管肺炎,而RSV感染后所致的疾病主要是毛细支气管炎。
     结论
     1、小儿呼吸道感染部分与WUPyV感染有关。
     2、WUPyV感染的临床特点是:以1—3岁年龄组儿童常见,在春季多见,其临床表现与普通的支气管肺炎和支气管炎类似,少见重症表现。
     3、WUPyV合并其它病毒感染率高,常与RSV合并感染。
Objective To study the clinical features of respiratory infection caused by WU polyomavirus ( WUPyV) in children..
     Material and methods
     Material: From January 2008 to September 2010, throat swab samples were collected from1701 children who were hospitalized with respiratory tract infection in Second Affiliated Hospital of Shantou University Medical College, and the clinical datas about WUPyV-positive cases or WUPyV-positive cases were collected including age, gender, symptoms and signs.
     Methods: 1、Multiplex PCR was applied to detect 10 kinds of viruses including WU Polyomavirus (WUPyV), respiratory syncytial virus (RSV), Boca virus (HBoV), adenovirus (ADV), rhinovirus (RhV), influenza virus A, B type (FluA, FluB), parainfluenza virus type 1,3 (PIV1, PIV3 ) and Human metapneumovirus (hMPV).
     2、To compare the clinical datas of WUPyV-positive cases with that of WUPyV-positive cases.
     Results
     1、There were 1215 positive samples (71.4%) in 1701 specimens, with 46 WUPyV-positive cases (2.7%) and 365 RSV-positive cases (21.5%). There were 22 cases (47.8%) and 88 cases (24.1%) of simple infection for WUPyV and RSV.
     2、The positive rate of WUPyV peaked in spring, while the positive rate of RSV reached its peak in winter; the average age of children with WUPyV infection was 26.23 months old, greater than that of children with RSV infection.
     3、Clinical characteristics of children infected with WUPyV and RSV are similar, but Coughing, wheezing were more common in children with RSV infection; There was no significant difference in the aspects of signs in the lungs, WBC count, CRP value, length of stay and severity of disease for both virus infection. The disease caused by WUPyV infection is mainly bronchial pneumonia, while the disease caused by RSV infection is mainly capillary bronchitis.
     Conclusions
     1、Respiratory tract infections in children were partly caused by WUPyV infection;
     2、WUPyV infection were more common in the 1-3 age group children; the positive rate of WUPyV peaked in spring,and its clinical manifestations are similar to that of normal bronchial pneumonia and bronchitis, but rare in serious cases.
     3、WUPyV infection has a higher rate of co-infection with other viruses, especially with RSV.
