玉屏风散免疫作用机制的研究

摘要
肺主气,外合皮毛。益气固表法通过补益脾肺之气而达固表御邪之功,体现了“正气存内,邪不可干”的原理,临床常用于脾肺气虚,表卫不固证。玉屏风散为该治法的代表方剂,对机体免疫系统有广泛的影响,临床疗效确切。本研究主要对玉屏风散免疫作用机制进行研究,并以此对益气固表法进行探讨。
     通过实验观察了玉屏风散总多糖在全方免疫调控中的地位和作用,以及方中组成各药总多糖的配伍意义。以玉屏风散总多糖为探针,观察其在不同给药途径和给药时相对小鼠免疫功能的影响,肠道吸收情况,探讨其免疫调控的方式和过程。通过环磷酰胺、全肠道外营养和风寒所致的黏膜免疫损伤模型,研究其对肠道和呼吸道黏膜免疫的影响。以经典的免疫增强剂云芝多糖作为对照,探讨玉屏风散多糖在增强黏膜免疫功能作用上的独特性。
     结果:1、玉屏风散方中多糖成分是其免疫调控作用的主要物质,不同药物来源的多糖成分对小鼠免疫功能有不同的影响。2、玉屏风散方中多糖成分对肠道黏膜免疫系统具有独特的作用机制和方式,主要通过肠道过程本身影响肠道黏膜免疫系统,通过全身黏膜系统的紧密关联影响呼吸道黏膜免疫,同时作用于全身特异性和非特异性免疫。3、玉屏风散方中多糖成分预防给药和治疗给药均可影响与黏膜免疫相关的多个指标和环节,且其预防给药时能使机体免疫机能处于一种较高程度的“稳定状态”,有效防御外界致病因素的侵袭。
     玉屏风散多糖肠道过程可能通过激活整个黏膜免疫系统,进而引起呼吸道黏膜免疫增强,发挥防御作用。黏膜免疫功能的提高是体现益气固表作用的重要方式。本研究为玉屏风散临床应用和相关中医药理论研究的深化提供了依据,为多糖类药物的作用途径研究提供了新的思路。
In traditional Chinese medicine, Lung is the most important organ of qi, which has close relationship to the surface of body. The YiQiGuBiao method can strengthen the body surface resistance by improving the qi in spleen and lung. In clinic, this method is often used either in qi deficiency of spleen and lung or the instability of surface defensive function. YuPingFengSan is the present prescription of YiQiGuBiao method. It affects on the immune system and has a certain effect. In this research we mainly study on the mechanism of the prescription and the YiQiGuBiao method.
    In the experiment we observed the function of YuPingFengSan total polysaccharide on immune regulation and the prescription composing meaning of each polysaccharide. Also we observed the effect of total polysaccharide on mice immune function during different administration or periods, and the absorption in intestine. We discussed its effect on intestinal and respiratory mucous membrane immunity, through the immune deficiency model caused by cyclophosphane, TPN and cold. The research took YunZhi polysaccharide the immunity enhancer as control, to discuss the speciality on mucous membrane immune enhancement.
    Results: 1. The polysaccharide in YuPingFengSan is the main substance of immune regulation, and different polysaccharide in the prescription has different effect on mice immunity. 2. The polysaccharide in YuPingFengSan has unique mechanism and effect on intestinal mucous membrane immune system. The function is mainly produced by intestine progress itself and influence the respiratory mucous membrane, also it affects on specific and non-specific immunity of the whole body. 3. The polysaccharide in YuPingFengSan affects various indes and points in mucous membrane immunity, either by pre-administration or curing administration. Especially in the pre-administration way, the body immune faculty is kept in a high level "stability", which may be helpful to defend the invasion of outside factors.
    The intestinal progress of YuPingFengSan polysaccharide can activate the whole mucous membrane immune system to enhance the respiratory immunity. The enhancement of mucous membrane immunity is the key point in YiQiGuBiao method. This research discussed on the clinical use of YuPingFengSan and its related traditional Chinese medicine theoretic, also it suggested new researching direction of the polysaccharide drugs.
