快速康复外科在结直肠癌手术中的应用

摘要
前言
     自从上世纪90年代丹麦Kehlet最早提出了快速康复外科(fast track surgery,FTS)理念后,外科治疗理念就在逐步发生改变。FTS理念措施实施的目的是减轻围手术期病人的手术创伤,降低手术的应激反应,加快病人术后康复。本研究收集2009年1月至2009年10月期间收治的86例结直肠癌病人,分别应用FTS方案和传统对照方案,对两组病人的首次排气排便时间,术后住院费用,住院天数,术后并发症以及术后应激和免疫功能的恢复等进行对照研究。
     目的
     比较快速康复外科(FTS)治疗方案较传统方案治疗结直肠癌患者的优越性。
     方法
     对比研究86例结直肠肿瘤病人,分别采取快速康复外科治疗方案(n=31)和传统的治疗方案(n=55),比较两组病人术后住院时间、住院费用、首次排气排便时间和术后并发症的发生率,血清应激及免疫等临床指标。
     结果
     两组病人均痊愈出院。快速康复组病人与传统对照组相比,术后首次排气排便时间均明显提前,住院费用及住院时间明显减少(P<0.05)。FTS对应激及免疫功能的改善作用优于传统方法。但两组并发症比较无明显差异(P>0.05)。
     结论
     结直肠癌患者行快速康复外科治疗是安全、有效的;结直肠癌患者快速康复外科治疗方案与传统治疗方案相比,减少住院时间及住院费用;快速康复外科减轻术后切口疼痛,并有利于患者术后应激及免疫功能的恢复。
Preface
     Since the 90s of last century Danish Kehlet [1] first proposed a rapid recovery surgery (fast track surgery, FTS) concept, the concept of surgical treatment in progressive change. FTS measures to implement the concept of the purpose is to reduce perioperative surgical trauma patients, reducing the surgical stress response, accelerated postoperative recovery [2]. In this study from January 2009 to October 2009 period,86 cases of colorectal cancer treated patients, respectively, and traditional application of the control program FTS program, the two groups for the first exhaust defecation time, postoperative hospitalization costs, length of hospital stay, complications and postoperative stress and immune function recovery were studied as controls.
     Objective
     Fast-track surgery (FTS) treatments more traditional treatment of colorectal cancer patients scheme.
     Methods
     Comparative Study of 86 cases of colorectal cancer patients, fsts-track surgery were taken (n= 31) and traditional treatment programs (n= 55), postoperative hospital stay were compared, hospital charges, the first bowel movement and discharge patients The incidence of complications, stress and immune serum postoperatively.
     Results
     Two groups of patients were cured. Fsts-track surgery patients with the traditional group than the control group, after first bowel movement was significantly earlier discharge, hospital costs and length of stay decreased significantly (P<0.05). FTS on stress and improve immune function better than traditional methods. However, no significant difference between the two groups of complications (P> 0.05).
     Conclusion
     By fsts-track surgery of patients with colorectal cancer surgical treatment is safe and effective; fsts-track surgery in patients with colorectal cancer surgical treatment compared with conventional treatment to reduce the length of stay and hospital costs; fsts-track surgery to reduce postoperative pain, and facilitate Stress and immune function in patients with postoperative recovery.
