经穴与脏腑靶器官相关联聚焦性与弥散性功能效应特性规律的实验研究

摘要
目的:通过针刺背俞穴及其同名经原穴,探讨其对心脑靶器官特异性相关联的针刺效应规律和机理。
     方法:在心肌梗死和脑出血大鼠模型的制备中,分别随机选取80只健康大鼠。把各模型80只大鼠随机分为对照组、模型组、心俞组、神门组、肾俞组、太溪组、承山组和非经非穴组,每组10只大鼠。除对照组为假手术组外,其他各组均制备相应的冠脉结扎心梗大鼠模型和Rosenberg法脑出血大鼠模型。在针刺治疗七天后观察各组大鼠以下指标:(1)心电图。(2)血清心肌酶学。(3)心肌组织和脑组织的NO、NOS和SOD。(4)血清IL—2和IL—6。(5)心肌组织和脑组织的MMP2和MMP9。通过对以上特异性指标的观察来探讨经穴是否对心脑靶器官有针刺效应及聚焦性与弥散性功能效应特性规律。
     结果:在心肌梗死模型的心电图指标的观察中,与模型组比较心俞和神门组有显著差异,P分别<0.01和0.05;在心肌酶学指标的观察中,与模型组比较心俞、神门和肾俞组有显著差异,除CK指标的神门组P<0.05其他均<0.01;在NO和NOS指标的观察中,与模型组比较心俞、神门和太溪组有显著差异,除NO指标的太溪组P<0.05其他均<0.01:在MMP2和MMP9指标的观察中,与模型组比较心俞、神门和肾俞组有显著差异,除MMP2指标的肾俞组P<0.05其他均<0.01。在脑出血模型IL—2和IL—6指标的观察中,与模型组比较心俞和肾俞组有显著差异,P<0.05;在NO、NOS和SOD指标的观察中,与模型组比较心俞、肾俞和神门组有显著差异,除NOS指标的心俞组和SOD神门组P<0.05其他均<0.01;在MMP2和MMP9指标的观察中,与模型组比较心俞、肾俞和神门组有显著差异,除MMP9指标的心俞组P<0.05其他均<0.01。而在两模型的各个指标的观察中,与模型组比较承山组和非经非穴组均无显著差异。
     结论:相对于承山组和非经非穴组,针刺背俞穴心俞及其同名经原穴神门对心脏靶器官有显著穴位特异性规律;而针刺肾俞穴及其同名经原穴太溪对心脏靶器官仅有一般(相对)穴位特异性规律。
     相对于承山组和非经非穴组,针刺背俞穴心俞及肾俞穴对脑靶器官有显著穴位特异性规律:而针刺神门穴对脑靶器官仅有一般(相对)穴位特异性规律。
     与承山穴和双非穴的各个指标观察对比来看,针刺背俞穴心俞和肾俞穴对心脑靶器官有比较鲜明的聚焦性与弥散性功能效应特性规律。
Objective:To research effect of acupuncture and mechanism of heart and brain associated with target organ specificity through acupuncturing back-shu point and the same meridian original-point.
     Methods:In the rat model preparation of AMI and ICH,80 healthy rats were randomly selected distinctly.Both of the model were randomly assigned to the control group,model group,Xinshu group,shenmen group, Shenshu group,taixi Group,chengshan group and non-meridian-non-Point Group,each group has 10 rats.In addition to the control group in the sham-operated group,the other groups were prepared corresponding coronary artery ligation AMI model and Rosenberg ICH model.After seven days in the acupuncture treatment,each group rats were observed in following indicators:(1)ECG.(2)serum myocardial enzyme.(3) myocarditis and brain tissue NO,NOS and SOD.(4)serum IL-2 and IL-6. (5)myocarditis and brain tissue MMP2 and MMP9.Through the observation of mention-aboved specific indicators,to explore whether meridian point has law of acupuncture effects and focus or diffuse function effects to heart and brain target organ.
     Results:In ECG indicators of AMI model,compared with the model group, Xinshu and shenmen group have significant difference,respectively P<0.01 and 0.05;in NO and NOS indicators,compared with the model group, Xinshu,shenmen and taixi Group have significant difference,in addition to CK indicators shenmen group P<0.05 all other<0.01;in MMP2 and MMP9 indicators,compared with the model group,Xinshu,shenmen and shenshu guoup have significant difference,in addition to MMP2 indicators shenshu group P<0.05 all other<0.01.In IL-2 and IL-6 indicators of ICH model,compared with the model group,Xinshu and shenshu group have significant difference,P<0.05;in NO,NOS and SOD indicators, compared with the model group,Xinshu,shenshu and shenmen group have significant difference,in addition to NOS xinshu group and SOD shenmen group P<0.05 all other<0.01.In MMP2 and MMP9 indicators,compared with the model group,Xinshu,shenshu and shenmen group have significant difference,in addition to MMP9 indicators xinshu group P<0.05 all other<0.01.Than the two models in the various indicators of observation, compared with the model group,chengshan group and non-meridian-non-Point Group have no significant difference.
     Conclusions:Compared with chengshan group and non-meridian-non-Point Group,acupuncturing xinshu of back-shu point and shenmen of the same meridian original-point have significant point-specificity law to heart target organ,but shenshu and the same meridian original-point have the only general(relative)point-specificity law to heart target organ. Compared with chengshan group and non-meridian-non-Point Group, acupuncturing xinshu and shenshu of back-shu point have significant point-specificity law to brain target organ,but shenmen point has the only general(relative)point-specificity law to brain target organ.And the chengshan group and non-meridian-non-Point Group observation comparison of the various indicators,acupuncturing xinshu and shenshu points of back-shu point have a clear focus and diffuse functional effects of law to heart and brain target organs.
