长沙市社区医务人员关于2型糖尿病干预的KAP调查研究

摘要
目的调查长沙市社区医务人员关于糖尿病干预的知识(K)、态度(A)、行为(P)的基本情况,探讨糖尿病干预知识的影响因素,为社区开展糖尿病干预及社区医务人员相关知识培训提供理论依据及实践指导。
     方法采用分层—整群抽样方法,随机抽取长沙市5个区的21家社区卫生服务中心的医务人员为研究对象,参与问卷调查。共发放问卷250份,回收250份,其中有效问卷234份,有效回收率93.60%。调查问卷包括一般资料和自制的社区医务人员有关糖尿病干预的KAP调查表。采用spss13.0统计软件对资料进行统计学分析,统计方法包括描述性统计分析、两独立样本的t检验、完全随机设计的方差分析、秩和检验、pearson相关分析、多元逐步回归分析,等。
     结果1.社区医务人员糖尿病干预知识平均得分25.66分,不同年龄、学历、职称的社区医务人员对于糖尿病干预的知识掌握程度之间存在统计学差异(p<0.05)。
     2.社区医务人员糖尿病干预态度平均得分19.85分,糖尿病干预行为平均得分10.26分。
     3.pearson相关分析显示:糖尿病干预态度与知识(r=0.232)及糖尿病干预行为与知识(r=0.162)之间具有正向相关关系(p<0.05)。
     4.影响社区医务人员对于糖尿病干预知识掌握情况的主要因素是:态度(b′=0.235)、学历(b′=0.229)、是否主动为糖尿病患者做指导(b′=0.176)。
     结论1.总体来说,社区医务人员关于糖尿病干预的知识水平较低,对其具有影响的因素主要是:对糖尿病干预的态度、学历及是否主动为糖尿病患者进行指导。
     2.社区医务人员关于糖尿病干预态度平均得分较高,行为平均得分不能达到理想水平。
     3.社区医务人员对于糖尿病干预的知识与态度及行为之间存在正向相关关系。
Objective To investigate the basic situation of knowledge, attitude and practice(KAP) about the diabetes mellitus intervention (DMI) in Changsha communities medical personnel, discuss the relationship between these three variables as well as the influential factors about the DMI knowledge, and provide the theoretical basis and the practical guide to develop the DMI and the related training progammme for the communities medical personnel.
     Methods By stratified cluster sampling method, the medical personnel from 21 community health service centers in five districts of Changsha had been stochastically enrolled in this study. There were 250 volunteers who accepted the investigations and filled in the questionnaires, 250 questionnaires were retrieved, of which 234 effective records, the effective retrieve-ratio: 93.60%. The questionnaire was composed with two parts: the demographic information questionnaire and the self-designed questionnaire of medical staffs' KAP on DMI in Changsha communities. The statistical data were analyzed with the Statistical Package for Social Science (SPSS) 13.0, and the statistical methods used were: descriptive statistics, independent-samples t Test, completely random design analysis of variance, rank-sum test, Pearson correlation analysis and multiple stepwise regression.
     Results 1. On average, the medical staffs in communities scored 25.66 points on the knowledge of DMI. The knowledge on DMI in this population varied with age, educational background and professional title (p<0.05).
     2. The average score on attitude and practice of DMI were 19.85 and 10.26, respectively.
     3. Pearson correlation analysis showed that there were positive correlation between knowledge and attitude of DMI(r=0.232), as well as knowledge and practice of DMI(r=0.162) (P<0.05).
     4. The major factors influencing the knowledge of DMI for medical staffs in communities were attitude and educational background as well as whether they would initially guide the diabetic patients or not.
     Conclusion 1. On the whole, the medical staffs in communities were short of knowledge concerning DMI, and the major factors influencing the knowledge of DMI for them in communities were attitude and educational background as well as whether they would initially guide the diabetic patients or not.
     2. The average score on the attitude of DMI of the medical staffs was higher than the score of practice of DMI.
     3. As far as the medical staff in community are concerned, the knowledge, attitude and practice of DMI are positively correlated.
