国家基本药物制度供给与实践研究

摘要
2009年至今，我国已经建立国家基本药物制度并取得了一定成效，但是，在实施过程中，一些制度的缺陷也暴露出来，影响了卫生机构服务能力，无法满足群众的基本卫生服务需要，阻碍了“医药卫生体制改革”的持续推进，急需完善国家基本药物制度与群众的基本卫生服务需要相适应。
在对苏州地区调研的基础上，分析国家基本药物制度运行效果，发现基本药物遴选机制欠佳、基本药物采购机制落后、基本药物补偿机制不一等原因，造成基层卫生机构药物配备不足，基本药物价格虚高，影响基本药物公平获取等问题。
以新制度经济学制度供给与需求理论为基本分析范式，从制度供给意愿和能力两个方面对国家基本药物问题的根源进行理论分析，制度目标的笼统使得制度供给不适应制度需求，而制度环境改革滞后和制度设计缺乏创新增加了制度供给成本、降低了制度供给效率。
最后借鉴国外基本药物制度的先进经验，提出推动基本药物遴选治理、强化政府供应主导作用、加快相关配套制度改革、促进基本药物合理使用等改善国家基本药物制度的建议。
Since2009, National essential medicine institution has been established in Chinaand has achieved some success. However, some of the deficiencies in the institutionexposed in the implementation process, health institutions unable to meet the needs ofthe people's basic health services, hinderingthe continued progress of the medical andhealth system, there is an urgent need to improve the national essential medicineinstitution and people's basic health services need to adapt.
Based on analysis of the operating results of the national essential medicineinstitution in Suzhou found that poor selection of essential medicines mechanism,essential medicine procurement mechanisms behind different compensation mechanismof essential drugs and other reasons, resulting in the lack of grass-roots healthinstitutions drugs with essential medicine artificially high prices, the impact of equitableaccess to essential drugs and other issues.
New institutional economics system of supply and demand theory as the basicanalysis paradigm, from the willingness and ability of the system supply theoreticalanalysis of the root of the problem of national essential drug system target generalmakes the system supply suited to system requirements, and institutional environmentreform lags behind the lack of innovation and design of the system has increased thecost of the system supply, reducing the efficiency of the system supply.
Lastly, learn the advanced experience form international essential medicineinstitution, under the "new medical reform" and "building a harmonious society"background, put forward to promote the essential drugs selection governance,strengthen the role of the Census and supply-led, to speed up the reform of the relevantsupporting systems, to promote the rational use of essential drugs and otherrecommendations to improve the national essential drug system.
