乌司他丁治疗改善重症中暑后肺泡表面活性物质的减少

摘要
目的:观察乌司他丁对重症中暑急性肺损伤大鼠肺泡表面活性物质蛋白B(SP-B)和磷脂浓度的影响。方法:将60只)健康SPF级成年雄性SD大鼠建立重症中暑急性肺损伤模型,建模成功后随机分为研究1组、研究2组、研究3组,每组20只动物,分别给予研究1组大鼠尾静脉注射高剂量乌司他丁(20万U/kg),给予研究2组大鼠尾静脉注射低剂量乌司他丁(5万U/kg),给予研究3组大鼠注射等剂量生理盐水,分别于腹腔注射药物前及第30min、1h、2h每组各分批次处死5只大鼠,采用酶联免疫吸附法(ELISA)法检测肺组织中SP-B的表达水平,使用比色法测定支气管肺泡灌洗液中磷脂成分浓度。结果:注药前3组大鼠肺组织SP-B和磷脂浓度均无显著差异(P>0.05),注药后30min、1h、2h,研究1组大鼠肺组织SP-B和磷脂浓度均明显高于研究2组(P<0.05)和研究3组(P<0.01),研究2组大鼠肺组织中的SP-B和磷脂浓度亦高于研究3组大鼠(P<0.05)。结论:乌司他丁能够有效的延缓重症中暑大鼠模型肺组织中SP-B和磷脂肺泡表面活性物质的减少,对重症中暑所致急性肺损伤具有保护效果。
Objective to investigate the effect of ulinastatin on the expression of pulmonary surfactant protein B(SP-B) and phospholipid in the rats with acute lung injury induced by severe heat-stroke.Methods:Sixty healthy adult male SD rats were used to stimulate acute lung injury with severe heat-stroke.The animals were randomly divided into three groups after onset of heat-stroke(n = 20 in each group).High dose ulinastatin(200000 U/kg) were intravenously injected via tail vein in Group 1.Low dose ulinastatin(50000 U/kg) were intravenously injected in Group 2.Equal volume normal saline were injected intravenously in Group 3.The expression of SP-B in the lung tissue was mesured by enzyme-linked immunosorbent assay(ELISA) and the concentration of phospholipid in bronchoalveolar lavage fluid(BALF) was detected by colorimetry in five sacrificed rodents of every group before administration as well as 30 min,1h and 2h after administration.Results:The level of SP-B and phospholipid had no significant difference among the three groups before administration.(P > 0.05).The concentrations of SP-B and phospholipid in Group 1 were significantly higher than Group 2(P < 0.05) and group 3(P < 0.01) at 30 min,1 h and 2h after ulinastatin treatment Further,the production of SP-B and phospholipid in Group 2 were also more than Group 3 at the same time.(P < 0.05).Conclusions:Ulinastatin has a curative effect on acute lung injury caused by severe heat-stroke.These effect may be related to improve the decrease of SP-B and phospholipid in lung tissue of severe heat-stroke rats effectively
