数字化穿支皮瓣标本的设计与制作

摘要
目的探索数字化穿支皮瓣皮动脉血供,建立穿支皮瓣血管的数字化模型,为术前血管体表定位提供解剖学依据。方法选取6具新鲜成人标本分别采用羧甲基纤维素(carboxymethylcellulose,CMC)/氧化铅水凝胶及乳胶-羧甲基纤维素/氧化铅混合填充剂行一次性全身动脉造影,X线、CT扫描获取标本灌注前后及剥离皮肤序列图像数据,运用Mimics软件,在计算机上对各部重要结构进行血管三维立体重建及穿支皮瓣的显示。结果标本经X线扫描可得到清晰的动脉成像,利用Mimics17.0重建了骨骼、血管、皮肤数字化可视模型,能够清晰显头面部、躯干、四肢及手足等部位主要穿支皮瓣皮动脉的来源及其供血皮瓣的形态、分布范围、毗邻关系,该模型可任意方向旋转观察。结论以羧甲基纤维素/氧化铅水凝胶及乳胶-羧甲基纤维素/氧化铅混合填充剂的灌注技术可使X线摄影及CT扫描在标本上获得高质量的血管影像,并以此设计不同类型的皮瓣,有利于穿支皮瓣的发掘、血供评估,为数字化穿支皮瓣提供理想的技术支持。
Objective To explore the digitalized perforation flap pedicled arterial blood supply,and establishment of perforated flap vascular digital model for the preoperative vascular surface in oder to provide anatomical basis.Methods Six adult cadaver specimens were treated with carboxymethyl cellulose-lead oxide hydrogel and latex-carboxymethyl cellulose/lead oxide mixed filler and line one-time generalized arterial angiography,X-ray,CT scans were taken before and after perfusion of skin samples.Then,using Mimics software,reconstruction of the three-dimensional structure of the relevant three-dimensional vascular and perforation flap design on the computer.Results The arterial imaging was obtained by X-ray scanning and the digital visual model of bone,blood vessel and skin was reconstructed by Mimics17.0,which can clearly show the face,trunk,limbs and hands,foot and other parts of the main perforator skin flap and the source of the blood supply flap,distribution,adjoining relationship,the model can be rotated in any direction.Conclusion The perfusion technique of carboxymethyl cellulose/lead oxide hydrogel and latexcarboxymethyl cellulose/lead oxide mixed filler can make X-ray photography and CT scan to obtain highquality vascular images on specimens,and can design different types of flaps,so conducive to the design of the flap flap,blood supply assessment,for the digital perforating flap to provide the ideal technical support base.
