摘要
目的探讨抑郁症患者对情绪加工的偏侧化机制及疾病对大脑认知功能的影响,了解N170在抑郁症患者情绪加工过程中的临床评估作用。方法纳入抑郁症患者31例(15例处于缓解期,16例处于发作期),14例健康对照,采用自编问卷收集社会人口学信息,采用贝克抑郁量表第二版(Beck Depression Inventory-Ⅱ,BDI-Ⅱ)量表评估患者的抑郁程度,所有参与者完成结合后掩蔽的点探测任务,同时采用128导联EGI设备记录脑电,选取国际10-20系统中大脑后部左侧和右侧P7、P8区域电极分析事件相关电位中N170成分的波幅及潜伏期。结果在负性一致、负性不一致和中性试次条件下,抑郁缓解组、发作组和健康组按键反应时进行比较差异无统计学意义。呈现负性刺激面孔时,抑郁发作组的P8区域电极上N170波幅明显大于P7区域电极的N170波幅,差异具有统计学意义(P=0. 023);其他组的左右脑N170波幅差异无统计学。呈现中性刺激面孔时,抑郁发作组的P8区域电极上N170波幅明显大于P7区域电极的N170波幅,差异具有统计学意义(P=0. 008);其他组的左右脑N170波幅差异无统计学意义。结论大脑偏侧化效应是一种自下而上的加工机制,抑郁症发作期患者对情绪面孔加工存在大脑偏侧化效应。在未来的临床工作中,阈下刺激所诱发的N170成分可用于临床检查,为抑郁症的诊断提供客观的诊断依据。
        Objective To explore the lateralization mechanism of depression patients in emotional processing and understand the clinical evaluation of N170 in the emotional processing of depressive patients. Methods A total of 31 cases of depression( 15 cases of major depression in remission,16 cases of major depression in episode),and 14 healthy controls were recruited in this study. The general data collection and Beck Depression Inventory-Ⅱ( BDI-Ⅱ) assessment were carried out for all participants. All participants completed the dot-probe tasks with post-masking. EEG data were recorded with EGI 128-channel system. At electrodes P7 and P8,the N170 amplitude and latency were analyzed. Results There was no significant difference in response time under negative consistency,negative inconsistency and neutral trial conditions among depression remission group,episode group and healthy group. When facial stimulus was negative,the N170 amplitude of P8 area electrode was significantly larger than that of P7 area electrode in depressive episode group( P = 0. 023). When facial stimulus was neutral,the N170 amplitude of P8 area electrode was significantly larger than that of P7 area electrode in depressive episode group( P = 0. 008). There was no significant difference in N170 amplitude between left and right brain in healthy group and depression remission group. Conclusion Brain lateralization is a bottom-up processing mechanism,which exists in emotional face processing in depressive episodes. Brain lateralization( N170) in patients with depressive episodes can be used in clinical diagnosis,and provide an objective basis for the diagnosis of depression in future clinical work.
