基于数据挖掘分析聂惠民治疗月经后期用药特色

摘要
目的:基于中医传承辅助系统分析聂惠民治疗月经后期的临床用药规律。方法:收集1999～2005年聂惠民治疗月经后期的病案,采用频次分析、关联规则及复杂系统熵聚类法等数据挖掘方法,确定处方中各种药物的使用频次、四气五味、归经以及关联规则,总结聂惠民治疗月经后期的用药规律及经验。结果:对筛选出的137个处方进行分析,处方中使用频次较高的药物有当归、柴胡、川芎、赤芍、熟地黄、生地黄、炒白芍、牡蛎、党参、龙骨等,药物四气以寒、温、平为主,五味以苦、甘、辛居多,主要归肝、脾、心、肾经,常用的药物组合有川芎、当归,当归、柴胡,赤芍、川芎,赤芍、当归,熟地黄、当归等。依据药物之间的关联规则,挖掘出蝉蜕、桑叶、桑白皮等5组核心组合及由此构成的5首新处方。结论:聂惠民尤善于从郁论治,解郁调经法为治疗月经后期的基本方法,灵活运用经方,常用经方合方加减治疗。
Objective:To analyze NIE Huimin's clinical medication rules in the treatment of delayed menstruation based on traditional Chinese medicine inheritance support system. Methods:NIE Huimin's cases of delayed menstruation from 1999 to 2005 were collected. Data mining methods such as frequency analysis,association rules and entropy clustering for complex systems were used to determine the frequency of various herbs in prescriptions,four kinds of qi and five tastes,channel tropism and association rules. NIE Huimin's medication rules and clinical experience in treating delayed menstruation were summarized. Results:Total 137 prescriptions were selected and analyzed. The most frequent herbs include:Danggui(Angelicae Sinensis Radix) and Chaihu( Bupleuri Radix),Chuanxiong(Chuanxiong Rhizoma),Chishao(Paeoniae Radix Rubra),Shudihuang(Rehmanniae Radix),Shengdihuang(Rehmanniae Radix),Chaobaishao(Paeoniae Radix Alba),Muli(Ostrea Gigas Thunberg),Dangshen(Codonopsis Radix) and Longgu(OsDraconis) etc. The main four properties of these herbs were cold,warm,neutral and five tastes were bitter,sweet and pungent and belonged to liver,spleen,heart and kidney meridians. Commonly used combination herbs were Chuanxiong and Danggui,Danggui and Chaihu,Chishao and Chuanxiong,Chishao and Danggui,Shudihuang and Danggui,etc. According to association rules of herbs,total 5 core combinations,which were Chantui(Cicadae Periostracum),Sangye(Mori Folium),Sangbaipi(Mori Cortex),etc,and 5 new prescriptions were formed. Conclusion:Professor NIE Huimin is good at treating delayed menstruation from depression and uses relieving depression and regulating menstruation as the basic principle,classical prescriptions and prescription combination are used to treat the disease.
