初探“清上畅中法”在痤疮治疗中的运用

摘要
痤疮为皮肤科常见疾病,好发于青壮年群体,古今医家常以火、热论之,泻火、清热是其主要治法。而笔者经多年临床观察发现,现代青壮年痤疮患者由于受阳盛体质、饮食不节、起居失常等因素影响,往往上焦肺热、中焦脾胃失调的病理状态同时存在。临床可见皮肤油腻,颜面、颈项、胸背部散在粉刺、丘疹、脓疱、囊肿、结节等损害。伴口臭、大便黏滞、排便不畅等症状。若仅以热论之恐失偏颇,仅以清热法治之则难奏全功。故试分析此类痤疮的病因病机及特点,提出清上焦肺热、畅中焦气机的并治之法,自拟清金畅中饮运用于临床,使上焦热清、中焦湿化,从而达到改善皮损的目的。
Acne occurs with greatest frequency among the young adults. We usually think the heat is the cause of the disease. Discharging fire and clearing heat is the most common treatment method. During my clinical experience,acne among the young adults manifesting the lung heat and the spleen and stomach discord due to constitution with overabundance of Yang and irregularity in daily life and meal. Acne is characterized by comedones,papules,pustules and nodules affecting primarily the face,neck and upper trunk or accompany with sebum secretion increases,halitosis,abnormal defecation,etc. If only talking about the heat is the cause of the disease and clearing heating is the method,it is difficult to get perfect effect. If only treating the acne with clearing heat,it is hard to cure. The article is trying to analyze the cause of the acne form many ways. The article also provides clearing the lung and regulating the middle energizer to alleviate the disease. According to the situation,I homemade prescribe "Qingjin Changzhong Decoction"to improve the acne.
