鼻翼软骨移植在鼻尖肥大整形术中的应用

摘要
目的:探讨鼻翼软骨移植在鼻尖肥大整形术中的应用效果。方法:选取2016年8月-2018年6月在笔者医院行鼻尖肥大整形术的患者86例,采用随机数字表法将患者分为实验组和对照组,各43例。实验组给予自体鼻翼软骨移植,对照组给予膨体聚四氟乙烯假体填充。随访6个月,比较两组手术效果、术后鼻部美容效果、手术前后鼻部生理功能及并发症发生情况。结果:两组手术时间及鼻尖矫正成功率比较,差异均无统计学意义(P>0.05);实验组患者满意度95.35%明显高于对照组的76.74%,差异有统计学意义(P<0.05)。与术前比较,术后两组患者鼻尖高度、鼻长均明显增加,且实验组明显高于对照组,差异有统计学意义(P<0.05);两组鼻尖两顶尖距离均明显缩短,且实验组明显短于对照组(P<0.01);实验组术后鼻尖突出度及鼻背抬高度均明显高于对照组(P<0.01)。手术前后两组患者鼻腔纤毛运动速率及鼻腔阻力均无明显变化,组间比较差异无统计学意义(P>0.05)。术后实验组并发症发生率为2.33%(1/43)低于对照组的16.28%(7/43),差异有统计学意义(P<0.05)。结论:自体鼻翼软骨移植可更明显地调整鼻尖肥大患者鼻部形态,改善面部美观,提高患者满意度,且不影响患者鼻部生理功能,安全性高。
Objective To investigate the influence of alar cartilage transplantation on postoperative nasal physiological function and cosmetic effect in patients with nasorostral hypertrophy rhinoplasty. Methods 86 patients who were underwent nasorostral hypertrophy rhinoplasty in our hospital from August 2016 to June 2018 were selected and divided into the experiment group and the control group according to the random number table method, 43 cases in each group. The experiment group was given autologous alar cartilage transplantation, the control group was filled with expanded polytetrafluoroethylene prosthesis.Followed up 6 months, the surgical effects, postoperative nasal cosmetic effects and nasal physiological functions before and after surgery and complications were compared between the 2 groups. Results There was no significant difference in operation time and success rate of nasal tip correction between the two groups(P>0.05). The satisfaction rate of the experimental group was 95.35%, which was significantly higher than that of the control group(76.74%), the difference was statistically significant(P<0.05). Compared with pre-operation, the height of nasal tip and the length of nose in both groups increased significantly after operation, and the experimental group was significantly higher than the control group(P<0.05).The distance between nasal apex and nasal apex in both groups was significantly shortened, and the distance between nasal apex and nasal apex in the experimental group was significantly shorter than that in the control group(P<0.01).Postoperative nasal tip protrusion and nasal dorsal elevation in the experimental group were significantly higher than those in the control group(P<0.01).There was no significant difference in nasal ciliary movement rate and nasal resistance between the two groups before and after operation(P>0.05). The incidence of complications in the experimental group was 2.33%(1/43) lower than that in the control group(16.28%, 7/43, P<0.05). Conclusion Autologous alar cartilage transplantation can significantly adjust the nose shape of patients with nasorostral hypertrophy, improve facial appearance, enhance the satisfaction of patients, and it does not affect the patient's nasal physiological function, it has high safety.
