双顶径与孕龄对照表对唐氏综合征筛查整体效果和孕妇个体风险率变化的影响

摘要

目的分析双顶径与孕龄对照表对唐氏综合征筛查整体效果及孕妇个体风险率的影响。方法通过双顶径与孕龄对照表对2010年1月-2016年12月来该院进行产前筛查的3 253例孕中期孕妇的孕龄进行估计,并算出各对照表的产前筛查风险值,计算检出率、阳性率以评定唐氏综合征筛查整体效果,算出孕妇个体风险率以评定孕龄偏差对其个体的影响。结果在相同75%检出率的情况下,假阳性率最高为《中华妇产科学》对照表,其截断值为1∶325,而最低为宁波市某医院自制对照表,其截断值为1∶260。而孕妇个体风险值上,估计的孕龄小于真实值时,假阴性的风险提高;估计的孕龄大于真实值时,假阳性的风险提高。结论双顶径与孕龄对照表对唐氏综合征筛查存在不同的整体效果,宁波市某医院自制对照表进行孕龄估计的筛查效果较佳,其次是香港特别行政区对照表,同时,偏差较大的孕龄估计对孕妇个体风险存在较大影响。
