我院门诊超说明书用药情况分析与分级管理

摘要
目的分析我院门诊的超说明书用药情况与分级管理措施。方法于我院门诊随机抽取1 980份用药处方,依据药品说明书以及相关诊疗指南分析处方是否存在超说明书用药情况,并采用分级管理方式进行管理。结果本研究纳入的1 980份处方中,共337份处方存在超说明书用药情况,占17.02%。其中涉及营养药、抗感染药物、外用药等多种药品,以超剂量用药、超适用人群用药最为常见,分别占总数的28.78%与26.41%。超说明书用药情况在儿科、耳鼻喉科、妇产科的发生率较高,分别占26.41%、22.26%、13.06%。结论我院存在较多的超说明书用药情况,采用分级管理模式对超说明书用药进行严格的规范化管理可提高患者用药的安全性,降低治疗风险。
Objective To analyze the hierarchical management measures and drug use beyond the instructionsin outpatient clinic of our hospital. Methods A total of 1 980 prescriptions were randomly selected from the outpatient clinic of our hospital. According to the drug instructions and the relevant diagnostic and therapeutic guidelines, the situation of drug use beyond the instructions was analyzed, and the prescriptions were managed by hierarchical management. Results Of the 1 980 prescriptions included in this study, 337 prescriptions existed drug use beyond the instructions, accounting for 17.02%.Among them, nutritional drugs, anti-infective drugs, external medicines and other drugs are involved. Drug use in excess of dosage and in excess of applicable populationover were the most common, and accounting for 28.78% and 26.41%respectively. The incidence of drug use beyond the instructions were higher in pediatrics, otorhinolaryngology, gynecology and obstetrics department, accounting for 26.41%, 22.26% and 13.06% respectively. Conclusion There are many cases of drug use beyond the instructions in our hospital. The strict and standardized management for drug use beyond the instructions by hierarchical management model can improve the safety of drug use and reduce the risk of treatment.
