初治肺结核患者外周血Th22和Th17细胞及其细胞因子水平变化

摘要
目的探讨Th22细胞和Th17细胞及其细胞因子水平在初治肺结核患者中的变化。方法选取80例初治肺结核患者,根据有无空洞分为有空洞组(n=33)和无空洞组(n=47),选取健康体检者76例为对照组,对所有入选者进行Th17细胞和Th22细胞数量及血浆IL-22和IL-17A浓度等测定。结果与对照组相比,初治肺结核患者外周血Th22细胞、Th17细胞数量、血浆IL-22和IL-17A水平均降低(P<0.05);有空洞组外周血Th22细胞、Th17细胞数量、血浆IL-22和IL-17A水平均低于无空洞组(P<0.05);Pearson相关提示在初治肺结核患者中,Th22细胞数量与Th17细胞数量呈正相关(r=0.286,P=0.006),血浆IL-22水平与Th22细胞数量、Th17细胞数量均呈正相关(r值分别为0.295、0.193,P<0.05)。结论外周血Th22和Th17细胞及其细胞因子水平与肺结核的发病和严重程度密切相关,对结核免疫反应起重要调节作用。
        Objective To explore the changes of T helper(Th) 22 and Th17 cells and their cytokines in the pathogenesis of initial treatment pulmonary tuberculosis. Methods A total of 80 pulmonary tuberculosis patients with initial treatment were enrolled in the study. They were divided into cavitations group(n=33) and non cavitations group(n=47). 76 healthy people were selected as control group. Their frequencies of Th22 and Th17 cells in peripheral whole blood and interleukin(IL)-22 and IL-17 A in plasma were determined. Results The percentage of Th22 and Th17 cells, plasma IL-22 level and plasma IL-17 A level in pulmonary tuberculosis patients with initial treatment were all lower than the control group(P<0.05). The percentage of Th22 and Th17 cells, plasma IL-22 level and plasma IL-17 A level in cavitations group were all lower than that in non cavitations group(P<0.05). There was a positive correlation between circulating Th22 and Th17 cells in pulmonary tuberculosis patients with initial treatment(r=0.286, P=0.006). Plasma IL-22 level was positively correlated with Th22 cells and Th17 cells(r=0.295, r=0.193, P<0.05). Conclusion Th22 and Th17 cells and their cytokines are correlated with pathogenesis and progression of pulmonary tuberculosis. They can play important roles in immune function of patients with pulmonary tuberculosis.
