雌三醇联合生物反馈电刺激配合盆底肌训练治疗更年期压力性尿失禁的疗效观察

摘要
目的研究雌三醇联合生物反馈电刺激配合盆底肌训练治疗更年期压力性尿失禁(SUI)的疗效。方法将2016年1月-2017年12月就诊于该院的90例更年期压力性尿失禁患者纳入研究,随机分为给予欧维婷雌三醇乳膏联合生物反馈电刺激配合盆底肌训练治疗的观察组和仅给予生物反馈电刺激配合盆底肌训练治疗的对照组,每组45例。观察两组盆底肌肉张力、漏尿量和治愈率指标。并填写尿失禁生活质量问卷(I-QOL),比较两组生活质量评分情况。结果治疗6个月后,观察组患者的阴道静息压、阴道收缩压、阴道收缩持续时间、生活质量评分表的限制性行为维度及影响社会心理维度均高于对照组(均P<0.05);治愈率高于对照组(P<0.05);尿失禁量低于对照组(P<0.05)。结论雌三醇联合生物反馈电刺激配合盆底肌训练治疗能有效改善更年期压力性尿失禁的症状。
Objective To research the curative effect of estriol combined with biofeedback electrical stimulation and pelvic floor muscle training in treatment of menopausal stress urinary incontinence(SUI).Methods From January 2016 to December 2017,90 patients with menopausal SUI were selected from Nanchang Municipal Maternal and Child Health Care Hospital and randomly divided into two groups.The patients in observation group were treated by estriol cream combined with biofeedback electrical stimulation and pelvic floor muscle training,and the patients in control group were treated by biofeedback electrical stimulation and pelvic floor muscle training,45 patients in each group.Pelvic floor muscle tension,urine leakage,and curative rates in the two groups were observed.Incontinence quality questionnaire(I-QOL) was filled in,the scores of life quality in the two groups were compared.Results After six months of treatment,vaginal resting pressure,vaginal systolic blood pressure,vaginal contraction duration time,restrictive behavior dimension and influence social psychological dimension of life quality score in observation group were statistically significantly higher than those in control group(P<0.05).The curative rate in observation group was statistically significantly higher than that in control group(66.7%)(P< 0.05).The amount of urinary incontinence in observation group was statistically significantly lower than that in control group(P<0.05).Conclusion Estriol combined with biofeedback electrical stimulation and pelvic floor muscle training can effectively improve the symptoms of menopausal stress urinary incontinence.
