摘要
目的:探讨单绒毛膜双羊膜囊双胎选择性胎儿生长受限(sFGR)的临床特征及产前超声监测价值研究。方法:随机选取2014年9月到2017年9月于我院确诊的单绒毛膜双羊膜囊双胎孕妇89例为研究对象,根据胎儿是否存在选择性生长受限分为受限组(42例)和非受限组(47例)。回顾性分析所有孕妇及胎儿的临床资料及超声检查结果,对比两组胎儿死亡率、并发症发生率,同时分析不同胎儿结局与相应分型及各项监测指标的相关性,并做统计学分析。结果:受限组胎儿死亡率更高(P<0.05),而两组在并发症发生率差异方面并无统计学意义(P>0.05);死亡组sFGR Ⅱ型人数占比显著高于存活组,差异有统计学意义(P<0.05);sGFR胎儿死亡与胎儿脐带附着点相关,中央附着点胎儿存活率显著高于帆状附着点胎儿,差异有统计学意义(P<0.05)。结论:单绒毛膜双羊膜囊双胎选择性胎儿生长受限实施产前超声监测有利于早期诊断并及时采取干预措施,提高胎儿存活率,降低妊娠并发症率,值得临床推广使用。
        Objective: To investigate the clinical characteristics and value of prenatal ultrasound monitoring in monochorionic diamniotic twins pregnancy selective fetal growth restriction. Methods: A total of 89 patients with monochorionic diamniotic twins pregnancy who were treated in our hospital from September 2014 to September 2017 were randomly selected for the study. According to whether fetal growth restriction exists, they were divided into restricted group(42 cases) and non restricted group(47 cases). The clinical data of all pregnant women and fetuses and the results of ultrasonic examination were analyzed retrospectively. The mortality of the two groups and the incidence of complications were compared, and the correlation between the different fetal outcomes and the corresponding typing and the monitoring indexes were analyzed, and the statistical analysis was made. Results: The fetal mortality in the limited group was significantly higher than that in the unrestricted group(P<0.05). There was no significant difference in the incidence of complications in the two groups(P>0.05), and the proportion of sFGR II in the death group was significantly higher(P<0.05); sGFR fetal death and fetal umbilical cord attachment point were compared. The fetal survival rate of the central attachment point was significantly higher than that of the velum attachment fetus(P<0.05). Conclusion: Prenatal ultrasound monitoring in monochorionic diamniotic twins pregnancy selective fetal growth restriction is beneficial to early diagnosis and timely intervention measures to improve fetal survival and reduce complications of pregnancy, which is worthy of clinical use.
