沙美特罗替卡松粉吸入剂联合丹参川芎嗪注射液治疗肺间质纤维化的疗效及对患者生活质量的影响

摘要
目的:探讨沙美特罗替卡松粉吸入剂联合丹参川芎嗪注射液治疗肺间质纤维化的疗效及对患者生活质量的影响。方法:选取2016年5月至2018年3月邢台市人民医院收治的肺间质纤维化患者98例,按照随机数字表法分为观察组和对照组,每组49例。对照组患者给予沙美特罗替卡松粉吸入剂治疗,观察组患者在对照组的基础上加用丹参川芎嗪注射液。观察两组患者的临床疗效,治疗前后的肺功能指标改善情况、血气指标[血氧分压(PaO_2)、血二氧化碳分压(PaCO_2)]水平、肿瘤坏死因子α(TNF-α)水平、转化生长因子-β1(TGF-β)水平及生活质量评分。结果:观察组患者的总有效率为85.7%(42/49),明显优于对照组的71.4%(35/49),差异有统计学意义(P<0.05);治疗后,观察组患者肺功能指标水平改善情况明显优于对照组;PaO_2水平明显高于对照组,PaCO_2水平明显低于对照组;血清TNF-α、TGF-β1水平明显低于对照组;心理状况、活动受限及症状评分明显低于对照组,上述差异均有统计学意义(P<0.05)。结论:沙美特罗替卡松粉吸入剂联合丹参川芎嗪注射液治疗肺间质纤维化的疗效理想,可改善患者生活质量。
OBJECTIVE: To investigate the efficacy of salmeterol xinafoate and fluticasone propionate powder for inhalation combined with salviae miltiorrhizae and ligustrazine hydrochloride injection in treatment of pulmonary interstitial fibrosis and its effects on patients' quality of life. METHODS: Totally 98 patients with pulmonary interstitial fibrosis admitted into Xingtai People's Hospital from May 2016 to Mar. 2018 were extracted to be divided into the observation group and the control group via the random number table, with 49 cases in each group. The control group was given salmeterol xinafoate and fluticasone propionate powder for inhalation, while the observation group additionally received salviae miltiorrhizae and ligustrazine hydrochloride injection on the basis of the control group. The clinical efficacy, improvement of lung function indexes, blood gas index(PaO_2, PaCO_2), tumor necrosis factor α(TNF-α), transformation and growth factor β1(TGF-β) and quality of life scores were observed before and after treatment. RESULTS: The total effective rate of the observation group was 85.7%(42/49), significantly better than that of the control group 71.4%(35/49), the difference was statistically significant(P<0.05). After treatment, the improvement of lung function indexes in the observation group were significantly better than those in the control group; the level of PaO_2 was significantly higher than that of the control group, and the level of PaCO_2 was significantly lower than that of the control group; the levels of serum TNF-α and TGF-β1 were significantly lower than those of the control group; the psychological status, activity limitation and symptom score were significantly lower than the control group, and the above differences were statistically significant(P<0.05). CONCLUSIONS: The efficacy of salmeterol xinafoate and fluticasone propionate powder for inhalation combined with salviae miltiorrhizae and ligustrazine hydrochloride injection in treatment of pulmonary interstitial fibrosis is ideal and can improve patients' quality of life.
