肿瘤标志物SCC-Ag、Cyf21-1、CEA、ProGRP、及NSE联合检测在肺癌诊断中的应用

摘要
目的通过体外分析检测血清中鳞状细胞癌抗原(SCC-Ag)、细胞角蛋白19片段抗原(Cyf21-1)、癌胚抗原(CEA)、胃泌素释放肽前体(ProGRP)及神经元特异性烯醇化酶(NSE)5种肿瘤标志物在健康体检者、肺良性疾病和肺癌人群中含量的差异,分析五种肿瘤标志物联合检测的可行性。方法回顾性分析2015年1月到2018年7月我院相关科室收治明确诊断肺癌患者90例,肺良性病变患者90例,体检中心健康体检者90例。通过收集三组数据进行比较分析,对比5种肿瘤标志物独立诊断与联合诊断肺癌的敏感性和特异性。结果三组SCC-Ag、Cyf21-1、CEA、ProGRP、NSE含量差异具有统计学意义(P<0.05),健康对照组与肺癌组对比SCC-Ag、Cyf21-1、CEA、ProGRP、NSE含量差异具有统计学意义(P<0.05),肺良性病变组与肺癌组对比五种标志物含量差异具有统计学意义(P<0.05),小细胞肺癌组ProGRP、NSE水平明显高于非小细胞肺癌组差异具有统计学意义(P<0.05);SCC-Ag、Cyf21-1、CEA、ProGRP、NSE联合检测敏感性高于单独检测,但是联合检测特异性下降。结论 SCC-Ag、Cyf21-1、CEA、ProGRP、NSE在肺癌患者、健康体检者以及肺良性病变患者体内含量差异较为明显,五种肿瘤标志物的联合检测有利于肺癌早期鉴别诊断。
        Objective Detection of five types of tumors in serum:SCC-Ag,Cyf21-1,CEA,proGRP and NSE Differences in levels of markers in healthy health checkups,lung disease and lung cancer,The feasibility of joint detection of five tumor markers.Methods Retrospective analysis of 90 cases of lung cancer,90 cases of lung benign lesions,and 90 cases of physical examination center health examination from January 2015 to July 2018.Comparative analysis by collecting three sets of data,Sensitivity,specificity and accuracy of independent and combined diagnosis of five tumor markers in lung cancer.Results The contents of SCC-Ag,Cyf 21-1,CEA,ProGRP and NSE in three groups were statistically significant(P<0.05),The difference of SCC-Ag,Cyf 21-1,CEA,ProGRP and NSE was statistically significant in the control group and lung cancer group(P<0.05).The difference between lung benign lesions and lung cancer was statistically significant(P<0.05),ProGRP and NSE levels were significantly higher in small cell lung cancer group than in non-small cell lung cancer group(P<0.05),SCC-Ag,Cyf 21-1,CEA,ProGRP,NSE joint detection sensitivity are higher than individual detection,But the combination of tests showed a decrease in specificity.Conclusion SCC-Ag,Cyf 21-1,CEA,ProGRP,and NSE showed significant differences in the levels of lung cancer patients,health checkups,and lung benign lesions.The combined detection of five tumor markers was beneficial to the early differential diagnosis of lung cancer.
