机采血小板红细胞溢出(冲红)原因及干预措施

摘要
目的探讨机采血小板红采集过程中细胞溢出的原因,并进行干预,减少红细胞溢出,提高血小板质量。方法将出现红细胞溢出现象的33名献血者作为溢出组,随机抽取同期33名未发生红细胞溢出现象的献血者作为对照组,分别抽取两组静脉血,对红细胞、血小板诸参数进行比较,并根据结果进行干预。结果比较有红细胞溢出的机采血小板献血者与未出现红细胞溢出的机采血小板献血者红细胞平均体积(MCV)、平均血红蛋白(MCH)及平均血红蛋白浓度(MCHC)的检测结果,其差异有统计学意义(P<0.05);白细胞(WBC)、血红蛋白(Hb)、红细胞压积(HCT)、血小板(PLT)、血小板分布宽度(PDW)、血小板比容(PCT)和平均血小板体积(MPV)检测结果的差异无统计学意义(P>0.05)。采取干预措施后,机采血小板发生红细胞溢出现象由2.1‰降低至0.7‰。结论献血者血液MCV、MCH及MCHC三项参数结果偏低可造成机采血小板采集过程中红细胞溢出,乳糜血、初次捐献血小板精神紧张及设备因素均可导致红细胞溢出。注重对机采血小板捐献者的筛选,具有良好的静脉穿刺技术,采集过程中要保证良好的采集流速,同时要加大机采血小板献血知识的宣传等措施,是减少红细胞溢出的重要方法。
        Objective To explore the causes of red blood cell overflow during the collection of apheresis platelets, and to intervene to reduce the overflow of red blood cells and improve the quality of platelet.Methods Totally 33 blood donors who had red blood cell spillover were used as spillover groups to randomly select 33 blood donors without red blood cell spillovers in the same period as the control group. Two groups of venous blood were extracted respectively, and the parameters of red blood cells and platelets were compared, and the results were intervened according to the results.Results The results of the detection of MCV, MCH and MCHC with red blood cell overflow and blood donors without red blood cell overflow were statistically significant(P<0.05), and there was no statistical significance(P>0.05) for the differences in the results of WBC, Hb, HCT, PLT, PDW, PCT and MPV. After intervention, red blood cell overflow of apheresis platelets decreased from 2.1‰ to 0.7‰.Conclusion The three parameters of blood donors' MCV, MCH and MCHC reducing can cause red blood cell overflow in the process of collecting platelets. Chylous blood, initial donation of platelet and equipment factors can lead to red blood cell overflow. It is an important method to reduce the overflow of red blood cells by paying attention to the screening of platelet donors, having good venipuncture techniques, ensuring good collecting velocity during the collection process, and increasing the knowledge of blood donation by apheresis platelets.
