凋亡抑制基因Livin在鼻咽癌中的表达及与预后的关系

摘要
目的探讨凋亡抑制基因Livin在鼻咽癌组织中的表达情况,分析其与鼻咽癌患者预后的相关性。方法选取南充市中心医院耳鼻喉科28例鼻咽癌患者的活检组织设为观察组,20例慢性鼻咽炎患者的鼻咽黏膜组织设为对照组,应用免疫组化技术SP法检测两组凋亡抑制基因Livin表达情况,并分析鼻咽癌组织中Livin表达水平与病理特征的关系,同时采用Cox回归模型分析影响患者预后的因素。结果观察组中Livin阳性表达率较对照组高(χ~2=6.700,P=0.010);鼻咽癌组织中Livin阳性表达率与T分期、N分期、临床分期、颈部淋巴结转移、远处转移有显著性差异(P<0.05);Livin阳性表达患者的1年生存率较Livin阴性表达者明显降低(χ~2=4.421,P=0.035),且经Cox回归模型分析发现,Livin表达、临床分期、N分期、远处转移是影响鼻咽癌患者1年生存率的独立危险因素。结论凋亡抑制基因Livin在鼻咽癌组织中呈高表达,其表达情况与鼻咽癌患者的预后密切相关,其有望成为评估鼻咽癌患者预后的分子标志物。
Objective To observe the expression of apoptosis suppressor gene Livin in nasopharyngeal carcinoma and analyze its correlation with prognosis of nasopharyngeal carcinoma. Methods 28 patients with nasopharyngeal carcinoma and 20 patients with chronic nasopharyngitis were selected from Department of ENT in our hospital and the nasopharyngeal mucosa tissues of these patients were taken as experimental subjects. Immunohistochemical technique SP was used to detect the expression of Livin in two groups. The relationship between the expression level of Livin and pathological features in NPC tissues was also analyzed. At the same time, Cox regression model was used to analyze the prognostic factors. Results The positive expression rate of Livin in nasopharyngeal carcinoma tissues was 67.86%, which was higher than 30% of chronic nasopharyngitis tissues(P=0.010). The positive expression rate of Livin in nasopharyngeal carcinoma tissues was significantly different from that in T stage, N stage, clinical stage, cervical lymph node metastasis and distant metastasis(P<0.05). The 1 year survival rate of Livin positive patients was 63.16%, and the 1 year survival rate of Livin negative expression patients was 100%. We can see that the 1 year survival rate of patients with Livin positive expression is significantly reduced(P=0.035). And Cox regression model analysis showed that Livin expression, clinical stage, N stage and distant metastasis were independent factors affecting the 1 year survival rate of nasopharyngeal carcinoma patients. Conclusion Apoptosis suppressor gene Livin is highly expressed in nasopharyngeal carcinoma, and its expression is closely related to the prognosis of nasopharyngeal carcinoma patients. It is expected to be a molecular marker to evaluate the prognosis of nasopharyngeal carcinoma.
