小剂量右美托咪啶对老年高血压患者围麻醉期血压的影响及安全性分析

摘要

目的研究小剂量右美托咪啶对老年高血压患者围麻醉期血压的影响及安全性。方法选取我院2017年3月～2018年7月行前列腺电切术的老年高血压患者80例,根据麻醉药物差异将患者分为研究组和对照组,每组各40例。对照组给予咪达唑仑静脉注射,研究组给予小剂量右美托咪啶。比较两组患者T_0、T_3期平均动脉压(MAP)、心率、血氧饱和度(SpO_2)与不良反应情况。结果两组T_0期MAP、心率、SpO_2比较,差异无统计学意义(t=0.126、-0.023、0.139,P>0.05);T_3期MAP、心率、SpO_2比较,差异均有统计学意义(t=4.154、2.754、-24.374,P<0.01)。研究组不良反应发生率为10.0%,明显低于对照组的27.5%(χ~2=10.051,P<0.01)。结论老年高血压患者围麻醉期应用小剂量右美托咪啶能平稳患者手术期间的血压、心率及血氧饱和度,且安全可靠。
        Objective To study the effect and safety of low-dose Dexmedetomidine on blood pressure of old patient with hypertension in perianesthesia. Methods 80 elderly patients with hypertension who underwent transurethral prostatectomy from March 2017 to July 2018 in our hospital were enrolled.and divided into study group and control group according to the difference of anesthesia drugs,40 cases in each group.The control group was given Midazolam with intravenous injection;the study group was given low doses Dexmedetomidine.The mean arterial pressure(MAP),heart rate(HR),oxygen saturation(SpO_2)and adverse reactions of two groups were compared in period of T_0 and T_3. Results There were no significant differences on the MAP,HR and SpO_2 between the two groups in T_0 period(t=0.126,-0.023,0.139,P>0.05);there were significant differences on the MAP,HR and SpO_2 between two groups in T_3 period(t=4.154,2.754,-24.374,P<0.01).The rate of an adverse event in the study group was 10.00%,which was significantly lower than that of control group(27.50%)(χ~2=10.051,P<0.01). Conclusion Low-dose of Dexmedetomidine can stabilize the blood pressure,heart rate and blood oxygen saturation of elderly patients with hypertension during the perianesthesia period,which is safe and reliable.
