间歇性导尿联合综合护理干预在腰椎术后尿潴留患者中的临床效果

摘要
目的探讨间歇性导尿联合综合护理干预在腰椎术后尿潴留患者中的临床效果。方法选取2017年1月～2018年9月我院收治的腰椎术后拔除尿管后出现尿潴留的40例患者作为研究对象,随机分为观察组和对照组,每组各20例。对照组采用常规尿潴留护理措施,观察组采用间歇性导尿联合综合护理干预。比较两组患者重置尿管、尿道损伤及尿路感染发生率;比较两组患者的导尿费用和护理时间;比较两组患者的护理舒适度。结果观察组的重置尿管和尿路感染发生率低于对照组,差异有统计学意义(P<0.05);观察组的护理舒适度优于对照组,差异有统计学意义(P<0.05);两组的尿道损伤发生率、导尿费用及导尿护理时间比较,差异无统计学意义(P>0.05)。结论腰椎术后引起尿潴留的患者使用间歇性导尿来恢复膀胱自主性排尿的方式安全有效,能减少重置尿管和尿路感染的发生,提高患者的护理舒适度。
Objective To explore the clinical effect of intermittent catheterization combined with comprehensive nursing intervention in patients with urinary retention after lumbar surgery. Methods A total of 40 patients with urinary retention after lumbar surgery in our hospital from January 2017 to September 2018 were selected, they were randomly divided into observation group and control group with 20 cases in each group. The control group was given routine nursing measures for urinary retention, while the observation group was given intermittent catheterization combined with comprehensive nursing intervention. The incidence of urethral injury and urinary tract infection were compared between the two groups. The cost of catheterization and nursing time were compared between the two groups. The nursing comfort of the two groups was compared. Results The incidence of urinary tract infection in the observation group was lower than that in the control group, the difference was significant(P<0.05). The nursing comfort of the observation group was better than that of the control group, the difference was statistically significant(P<0.05). There was no significant difference in the incidence of urethral injury, cost of catheterization and time of catheterization nursing between the two groups(P>0.05). Conclusion Intermittent catheterization is a safe and effective way to restore bladder autonomous urination in patients with urinary retention after lumbar surgery. It can reduce the occurrence of urinary tract infection and improve patients′ nursing comfort.
