非对称二甲基精氨酸、血管内皮生长因子与2型糖尿病合并大血管病变的相关性研究

摘要
目的探讨T2DM合并大血管病变患者血清非对称二甲基精氨酸(ADMA)和VEGF的水平及相关性。方法选择2016年6~12月于佳木斯附属第一医院内分泌科收治的T2DM患者80例,根据血管病变情况分为单纯糖尿病组(T2DM)和T2DM合并大血管病变组(MVC),每组40例。选取同期于我院健康体检者40名作为对照组(NC),测定各组FPG、HbA_1c、TG、TC等指标。ELISA测定血清ADMA和VEGF水平并进行相关性分析。结果 NC组、T2DM组、MVC组血清ADMA及VEGF水平逐渐升高[(39. 04±3. 66)vs(48. 32±5. 37)vs(63. 86±4. 44)μmol/L,(826. 43±132. 79)vs(961. 19±125. 25)vs(1486. 45±1051. 59)pg/ml,P<0. 01]。Pearson相关分析显示,ADMA与吸烟史、HbA_1c、LDL-C、TC、VEGF呈正相关;VEGF与吸烟史、LDL-C、ADMA呈正相关。Logistic回归分析结果显示,吸烟史、病程、ADMA、VEGF是T2DM合并大血管病变患者的危险因素(P<0. 01)。结论 T2DM合并大血管病变患者血清ADMA水平及VEGF水平均升高;ADMA与VEGF均与T2DM合并大血管病变相关。
        Objective To investigate the correlation between the levels of serum asymmetric dimethylarginine(ADMA)and vascular endothelial growth factor(VEGF)and macroangiopathy in patients with type 2 diabetes mellitus(T2 DM).Methods 80 T2 DM patients were recruited in endocrinology department of The First Affiliated Hospital of Jiamusi University from Jun 2016 to Dec 2016. These T2 DM patients were divided into groups without macrovascular complications(T2 DM,n=40)and group with macrovascular complications(MCV,n= 40). Healthy subjects were enrolled as normal control(NC,n=40). Serum levels of ADMA and VEGF were measured by ELISA.Results Compared with NC group,the levels of ADMA and VEGF were significantly increased in T2 DM and MCV group[(39. 04±3. 66)vs(48. 32±5. 37)vs(63. 86±4. 44)μmol/L,P<0. 01;(826. 43±132. 79)vs(961. 19±125. 25)vs(1486. 45±1051. 59)pg/ml,P<0. 01].The correlation analysis showed that serum ADMA were positively correlated with smoking history,Hb A1 c,TC,LDL-C and VEGF levels. Serum VEGF were positively correlated with smoking history,LDL-C and ADMA levels. Logistic regression analysis showed that smoking history,diabetes course,ADMA and VEGF levels were significantly associated with the development of macroangiopathy in MCV(P<0. 01).Conclusion Both ADMA and VEGF are associated with macrovascular disease in T2 DM patients.
