Purpose

Adiponectin (APN) is an adipose-derived plasma factor with anti-inflammatory and vascular protective properties. The aim of this study was to assess the circulating levels of APN across the lung circulation in patients with pulmonary arterial hypertension (PAH) and compare with controls subjects. APN was correlated with clinical, hemodynamic data and inflammatory cytokines.

Methods and Materials

Blood samples were obtained simultaneously from the radial artery (RA) and the pulmonary artery (PA) in patients with PAH (n=27) and chronic thromboembolic pulmonary hypertension (CTEPH) (n=7) during right heart catheterization. The control group included patients with Wolff-Parkinson-White syndrome (n=20), otherwise healthy. The level of APN was estimated by enzyme-linked immunosorbent assay.

Results

In patients with PAH and CTEPH plasma levels of APN in both RA and PA were higher as compared with controls (22.6 ¡À11.6 vs. 12.0¡À7.2 ¦Ìg/ml; p=0.001 and 25.2¡À13.8 vs. 12.3¡À7.0 ¦Ìg/ml; p< 0.001, respectively). There were no significant differences between the RA and PA levels of circulating APN in patients (p=0.14) and controls (p=0.759).There were moderate but significantly correlations with heart rate, right atrial pressure (RAP), NT-pro-BNP, NYHA functional class and proinflammatory cytokines (Table). Mean plasma APN levels in men were lower than those found in women (16.6¡À8.5 vs.26.2¡À11.8 ¦Ìg/ml; p=0.016).[table1]

Conclusions

It is unclear which role APN play in PAH. Increased levels of adiponectin may represent a physiological attempt to limit further endothelial damage in PAH patients.
